Report a Food or waterborne illness

If you think food or water has recently made you sick, report it here by answering a few questions about:

1.  Your symptoms

2.  Places you have recently visited

3.  Food you have eaten in the last few days


Telling us about your illness and sharing information about a food or water source you think made you sick will help us keep others in your community safe.  If a common source of illness is found, public health officials can take steps to prevent more people from getting sick.



Retail Food Establishment Complaint

If you would like to file a food safety complaint against one of the retail food establishments, grocery stores, convenience stores or mobile food facili​ties licensed by the Division of Food and Recreational Safety staff, please send your complaint to datcpfoodcomplaintsemergencyresponse@wisconsin.gov​ or call (608) 224-4714.

Include the following information in your email:

• Street Address of Establishment (if known)

• City/Town/Village*

• Date of Incident (mm/dd/yyyy)*

• Approximate Time Incident Occurred

• Description of What Happened*

• Any Additional Comments
