3. Food you have eaten in the last few days
Telling us about your illness and sharing information about a food or water source you think made you sick will help us keep others in your community safe. If a common source of illness is found, public health officials can take steps to prevent more people from getting sick.

If you would like to file a food safety complaint against one of the retail food establishments, grocery stores, convenience stores or mobile food facilities licensed by the Division of Food and Recreational Safety staff, please send your complaint to datcpfoodcomplaintsemergencyresponse@wisconsin.gov or call (608) 224-4714.
