Are You Up-to-Date on Your Shots?

The Wisconsin Immunization Registry, also called WIR, is an internet database that tracks vaccine records for Wisconsin residents.  Immunizations, also called vaccinations, are one of the greatest achievements in public health. Vaccines prevent disease in people who receive them. Additionally, if enough people in the community are vaccinated, the entire community can be protected because there is little opportunity for an outbreak to occur. Before vaccines, many children died from diseases like measles, pertussis (whooping cough), and Haemophilus influenza.

Tracking vaccines in the WIR helps prevent repeated immunization that may happen from visiting different doctors or missing or lost records.  Tracking you or your child’s vaccination record using this system will save you time, money, and hassle of increased vaccine appointments.  Although this program is online, the public does not have access to your records.  This program uses best practice to calculate a vaccination schedule, and helps you stay up to date with needed immunizations.    

To look up yours or your child’s immunizations:

  • Go to
  • Near the bottom of the page click on Public Immunization Record Access
  • Enter the first name and last name in the appropriate fields
  • Enter the birthdate MM/DD/YYYY
  • Enter either: social security number, Medicaid ID, or healthcare member ID
  • Click search and review the record 

When reviewing the Measles, Mumps, and Rubella (MMR) vaccine: adults born during or after 1957 who haven’t had measles or have never been vaccinated should get at least one dose of measles vaccine.  College students, international travelers, and health care personnel should get two doses at least 28 days apart according to the Wisconsin Department of Health Services. 

Some providers do not use the Wisconsin Immunization Registry, especially those from out of state.  If you have an immunization that you would like updated in the WIR please contact the Forest County Health Department at 715-478-3371 with the proof of immunization and we will gladly update your record!  If you have any questions on your immunization status please do not hesitate to reach out.
